Going through the eng and J version simultaneously you can tell the J is much harder. It’s hard to quantify at some points but most of the boss battles are much harder, with the bosses using their more devastating attacks more often and just generally being more aggressive (the fiends come to mind). Bosses also take less damage (Bolt2 on Kainazzo (in Baron) takes off about 3000 on the US version and 900 on the J). Leveling up seems to take a little longer too in the J, I’m not sure about that though.
Overall, the J version is better because of this. The challenge is just about perfect, as the US version is a little too easy for me, save for some stuff at the end like doing the Cave Baham early and the Zeromus fight. With the J version you have to be sharp early on in the game.
The added skills are sometimes useful, most notably Cecil’s Dark Wave attack. He loses HP from doing it however. Most of the other ability’s are just useless, but some of those weak cure-all ability’s like Prayer may be worth it at some points early on.
Also of note is the GBA Gameboy Advance version of Final Fantasy 4. There are some new things, like the translation and a few features. You can re-acquire some of the characters that are seriously wounded. So basically at the end you can switch in Yang, Cid, Palom, Porom, or Edward if you want. The best 5 is probably already being used though although Edge is really the only debatable one (imo). There is also another extra cave at the Mt. Ordeals for getting final weapons / armor for those characters above.
